Shipment of these items to the sampling site can also be arranged at an additional charge or the bottles can be picked up from the laboratory for sampling. INSTRUCTIONS

Chain Of Custody

Following the sampling instructions, all samples should be submitted in the proper conditions, accompanied by a properly completed Chain of Custody form with the following minimum information:

  • Client name, address, phone number and email address

  • Project name or sample site

  • Project Manager (Contact person receiving the report)

  • Name of sampler

  • Date and time of sampling

  • Type of samples

  • Analyses needed on each sample

  • Special Handling

  • Billing Information
